In the Room Where it Happens

I wonder how bad decisions are made. Not catastrophic ones, but just questionable ones. For example, was it an executive or a designer who thought it was a good idea that the picture of the French toast sticks on the front of the box required a beret and a twirly moustache drawn on them?
